Brain fog supplements: what has evidence, what doesn't

The causes to rule out first, then creatine, caffeine, omega-3, Semax and Bacopa ranked honestly.

01The short version

"Brain fog" is not a diagnosis, it is a description: slow thinking, poor recall, difficulty holding attention. Because it is a symptom with many causes, no single supplement fixes it, and the ones that help depend on what is behind yours. This guide runs through the causes worth ruling out first, then the supplements with evidence, ranked honestly.

02Rule these out first

Before spending money: sleep (under seven hours is the commonest cause by a distance), dehydration, iron or B12 deficiency (a blood test through your GP), thyroid function, undiagnosed sleep apnoea, and the medication list. Persistent fog that does not lift with sleep, or that started suddenly, is a GP visit rather than a supplement question. Fog that follows a viral illness sometimes needs weeks, not pills.

03Supplements with evidence, best first

1. Creatine

Not just for the gym. Several trials show improved working memory and reasoning, especially in people who are sleep-deprived or eat little meat. 3 to 5 g a day, cheap, safe, slow (weeks).

2. Caffeine with L-theanine

The fastest fix for alertness, and theanine smooths the edge. Does nothing for the underlying cause and stops working if you lean on it all day.

3. Omega-3 (EPA/DHA)

Modest benefit for cognition in people with low intake, essentially none in people who already eat oily fish.

4. Semax

For fog that is specifically fatigue-driven decline through the day, the 1996 study is directly relevant: attention and memory scores improved most in the most tired participants, and the effect lasted 20 to 24 hours [1]. It also raises BDNF, the growth factor behind learning [2][3]. It is a peptide nasal spray rather than a capsule, and it is what we make.

5. Bacopa monnieri

Reasonable trial data for memory after eight to twelve weeks of daily use. Slow, and it can upset the stomach.

6. Magnesium and B vitamins

Only if you are low. A multivitamin does not cure fog in someone who is replete.

04What does not have evidence

Lion's Mane in humans (mushroom marketing has outrun the trials), Ginkgo for healthy adults, "brain fog" blends with twenty ingredients at undisclosed doses, and anything sold with a before-and-after photo. Nootropic pouches and gums are caffeine with a new label.

05A sensible order to try things

  1. Fix sleep and get the blood test.
  2. Creatine daily for a month.
  3. Keep the first coffee, add L-theanine, drop the rest.
  4. If the pattern is "fine at 9, useless by 3", try Semax for a working month on the guarantee.
